setting is responsible for the evaluation of patients in relation to home assessment, functional status and mobility, safety _awareness,_
and other physical therapy needs basing

The Certified Occupational Therapist Assistant is responsible for collaborating with physician and medical care team in evaluating patient's level of function, determining treatment plan, provide skilled intervention as well as supervise personnel to ensure treatment goals are achieved to maximize and restore their patient's functional skills to the fullest in their home setting._

Must have a valid Illinois driver's license, vehicle insurance and dependable automobile. Or must be able to commute to the patient's location._

Must have a current CPR card._
