How You'll Make a Difference:

Arkos Health is seeking a
Care Navigator.

The Care Navigator will be responsible for facilitating care for patients by coordinating and scheduling with Arkos Health field providers and assigned primary care provider.

It is vital to foster goodwill and establish and retain positive, impactful, and successful relationships with the participating provider network, patients, health plan, and the organization.

The CN will possess excellent relationship building skills. This position requires someone with an outgoing and positive personality. The CN will maintain a collaborative, advising, and attentive attitude always. The CN will represent our organization with professionalism in behavior and dress.

The CN must be ready to work in a fast-paced environment, embedded in a provider's practice location(s), while also having the ability to work in multiple systems efficiently.


Essential Duties and Responsibilities:


  • Collaborates with the providers, Quality, Risk Adjustment, and Clinical teams to improve and support provider and patient engagement
  • Verifies eligibility and navigates through insurance portal(s)
  • Conducts telephone calls to patients, providers, and other health care professionals and organizations in support of care initiatives
  • Checkout patients and accurately and efficiently coordinates and tracks scheduling of member referrals and visits to be conducted by internal teams and/or primary care provider
  • Provide explanations of all Arkos Health patient services and obtain verbal consent from patients
  • Processes requests for member medical records if applicable and/or upon request
  • Processes requests for member medical records; Upload all orders, field documents, and requested records to the patient's charts for all internal teams and assigned primary care provider
  • Monitors, tracks, and reports patient data/outcomes accurately
  • Create reports for the monitoring and tracking of provider progress
  • Provides high quality customer service to both internal and external customers
  • Ensure that your supervisors are well informed of any provider or patient issues/concerns through consistent communication of outstanding issues in a timely fashion
  • Provide your supervisor/s with updates per their request
  • Play an active role in achieving organizational goals
  • Share improvement ideas with your supervisor/s
  • Utilize CarePointe Database and other organization systems' technology as dictated by your supervisor/s
  • Travel to local provider offices. Mileage reimbursement will be provided based upon policy
  • Work on special projects and other duties as assigned by management
  • Work collaboratively and constructively with the PR Team

Qualifications and Skills:


  • High School Diploma or GED equivalent required
  • Current unrestricted Arizona Driver's license, reliable transportation, and proof of insurance required
  • 2+ years' experience in a healthcare environment
  • RMA/CMA/CNA preferred
  • Preferred knowledge of medications, medical terminology, and patient care
  • Preferred knowledge of CPT, ICD, HCPCS, HCC coding, CMS Quality measures
  • Strong written and verbal communication skills
  • Strong skills in office equipment (computers, printers, etc.) and MS Office programs (Excel, Outlook, and Teams)
  • Ability to multitask and prioritize in a high stress environment and adapt to changing conditions and work assignments
