    Estates at Greeley - Minimum Data Set (MDS) Coordinator
    POSITION SUMMARY
    The MDS Coordinator oversees the completion, editing, locking, and transmission process of all MDS assessments to ensure accuracy, case mix maximization, and regulatory compliance. They coordinate care plan completion with the Interdisciplinary Team (IDT) and provide clinical support to enhance accurate assessments and care quality.
    ESSENTIAL RESPONSIBILITIES AND DUTIES
    RESPONSIBILITY: To ensure timely MDS completion focusing on accuracy, case mix, and compliance
    • Completes MDS based on various data sources
    • Updates resident information in the system
    • Completes Resident Assessment Protocol documentation
    • Works with IDT for comprehensive care plans
    • Submits completed MDS within specified time frames
    • Oversees necessary paperwork completion
    • Provides incidental direct care to monitor accuracy
    • Assists in staff training for improved care
    RESPONSIBILITY: Promotes Healthy Relationships and Professional Conduct with all parties
    • Arrives on time and prioritizes tasks efficiently
    • Maintains confidentiality and follows policies
    • Is courteous and cooperative with all individuals
    • Promotes a positive and professional image
    Knowledgeable about Monarch Healthcare Management policies and procedures.
    FACILITY COMPLIANCE
    • Promotes a positive workplace centered on customer service
    • Attends required education/in-services
    • Maintains knowledge of health regulations
    • Willingness to work evenings and weekends as needed
    QUALIFICATIONS
    Education and Experience
    • Must be a Registered Nurse (RN) in Minnesota
    • Certification in CPR and emergency care
    • Effective communication skills
    • Ability to perform job functions with or without accommodation
    • Pass criminal background study
    Preferred
    • Experience in healthcare settings
    • Experience with MDS and RAI
