- Deliver therapeutic treatments to individuals, groups, families, and/or significant others.
- Conduct individual treatment sessions based on program and patient requirements.
- Provide crisis intervention and ensure daily safety assessments.
- Interact respectfully with patients, families, significant others, and referents.
- Engage in psychoeducational support sessions with patients/families/significant others.
- Offer case management and therapeutic interventions to assigned patients.
- Act as a patient advocate in grievance situations, following System policies.
- Ensure clinical effectiveness and consistency in adherence to provided materials. Manage patient/family expectations regarding services and maintain communication with referring providers, schools, and the care team.
- Implement and personalize goals and interventions as per the Care Plan's therapeutic aspect.
- Ensure SMART goals (specific, measurable, attainable, relevant, and timely) are set.
- Document progress aligned with the treatment plan.
- Complete admissions documentation.
- Conduct Social Services Admission Assessment.
- Perform Suicide Risk Assessment and collaborate with the treatment team following specified guidelines.
- Develop safety plans when necessary.
- Establish initial treatment goals and interventions in the Care Plan.
- Obtain consent for release of records as needed for treatment facilitation or discharge.
- Maintain ongoing treatment documentation according to program guidelines.
- Create clear, concise treatment goals in the Treatment Plan.
- Daily documentation of patient's treatment status in the medical record.
- Document group treatment details and patient engagement. Update Care Plan goals and interventions accordingly.
- Record contact notes for conversations with outpatient providers, schools, or parents.
- Summarize patient progress and multidisciplinary team meeting outcomes in the master treatment plan after each official staffing.
- Coordinate treatment director's hold for Inpatient and Residential settings as required.
- Communicate processes to team members and ensure detailed, timely information documentation.
- Assist physicians with necessary documentation.
- Document initial discharge plan in the medical record.
- Prepare discharge summary detailing patient's presenting problem, progress, current status, and treatment recommendations.
- Coordinate with outpatient providers, schools, and stakeholders for aftercare discussions.
- Report and document suspected cases of abuse and neglect as per state laws and System policies.
- Demonstrate knowledge of System policies, procedures, and mandatory reporting requirements.
- Master's Degree in Social Work, or related field, from an accredited school of Social Work Professional Counseling, or Family Therapy preferred with healthcare and psychiatric setting experience.
- In-training as a Licensed Professional Counselor (LPC-IT), Marriage and Family Therapist Training License (MFT-TL), Social Worker Training Certificate (SWTC), Advance Practice Social Work (APSW) or equivalent in state practicing.
- SAC-IT, SAC, or CSAC required in WI. Substance abuse specialty authorization specific to the state of employment needed.
- Proficiency in therapeutic interventions for all age groups and human behavior dynamics.
- Completion of American Red Cross Professional Rescue and NCI training within specified timelines.
- Attend all mandatory and relevant in-services pertaining to the role.

The role of the Therapist involves developing, coordinating, and facilitating direct social services for patients and their families while following managerial directions. Operating in an acute care environment that encompasses partial hospitalization and intensive outpatient services, Therapists play a crucial part in guiding patients towards positive outcomes and transforming their narratives. This position entails delivering therapeutic treatments through individual, group, and family sessions, collaborating with a diverse treatment team to create patient care plans, and acting as a staunch advocate for our patients.
Job Duties & Responsibilities:
As outlined in the program description, responsibilities may entail cognitive behavioral therapies alongside social services.
Provide social services to patients and their families/significant others
Complete all documentation within required time
Complete discharge documentation as required and on time
