    Description

    Job Duties & Responsibilities:

    Per program description, this role may involve duties of cognitive behavioral therapies in addition to social services.

    Provide social services to patients and their families/significant others

    • Deliver therapeutic treatment services to individuals, groups, families, and/or significant others.
    • Conduct individual treatment sessions with patients as needed based on the program and patient requirements.
    • Offer crisis intervention services when necessary and conduct daily safety assessments.
    • Engage with patients, family members, significant others, and referents with respect and dignity.
    • Participate in psychoeducational support sessions with patients/families/significant others.
    • Provide case management and therapeutic interventions to all designated patients.
    • Act as a patient advocate in case of grievances, following the System's policies and procedures.
    • Adhere to provided materials to ensure clinical effectiveness and consistency. Manage patient/family expectations regarding services offered. Initiate and maintain communication with referring providers, schools, if applicable, and the care team for patients.
    • Implement specific goals and interventions as outlined in the Care Plan, within the therapeutic aspect of the patient's program.
    • Ensure goals are SMART (specific, measurable, attainable, relevant, and timely).
    • Document progress based on the treatment plan.

    Complete all documentation within the required timeframe

    • Finalize admissions documentation.
    • Conduct Social Services Admission Assessment.
    • Perform Suicide Risk Assessment and collaborate with the treatment team as per established guidelines.
    • Develop a safety plan (for patients aged 12 and under at Provider's discretion).
    • Establish initial treatment goals and interventions in the Care Plan.
    • Obtain necessary consent for release of records to access information aiding in treatment provision or discharge facilitation, including coordination with referents and other mental health treatment providers.
    • Complete ongoing treatment documentation as needed, in line with program guidelines.
    • Create clear, concise, collaborative, and achievable treatment goals in the Treatment Plan.
    • Record the patient's treatment status in the medical record daily.
    • Document therapeutic details for group treatments, including group descriptions and patient engagement/participation information. Update the Care Plan goals and interventions.
    • Record contact notes for each interaction with outpatient providers, schools, or parents.
    • Summarize the patient's progress and decisions from multidisciplinary team meetings in the master treatment plan during formal patient review sessions.
    • For Inpatient and Residential settings only: Assist with coordination of treatment director's hold as required and in line with program policy and guidelines.
    • Communicate the process to team members.
    • Thoroughly and promptly document all necessary information.
    • Assist the physician with essential documentation.

    Complete discharge documentation as necessary and on time

    • Record the initial discharge plan in the medical record.
    • Complete the discharge summary, detailing the patient's presenting problem, progress, current status, and treatment recommendations.
    • Contact outpatient provider(s), schools, and other stakeholders to discuss aftercare, ensuring a smooth transition.
    • Report and document suspected incidents of physical, sexual abuse, and/or neglect as per state laws and System's policies and procedures.
    • Adhere to System policies and procedures.
    • Understand and comply with Mandatory Reporting regulations in the state of employment.

    Provide leadership and guidance in implementing clinical interventions

    • Evaluate the outcomes of interdisciplinary interventions and adjust the Care Plan accordingly.
    • Assist in identifying and assessing treatment needs.
    • Participate in and/or lead interdisciplinary meetings for treatment planning, updates, and progress evaluation.

    Communicate professionally and accurately

    • Collaborate with and involve other departments and agencies as necessary and as outlined in Social Services policies.
    • Share relevant data with the attending provider and other team members, verbally or in writing as needed, distinguishing between situations requiring immediate attention and those that can be addressed later.
    • Promptly communicate challenging issues with referents, patients, family members/significant others, and colleagues.
    • Seek supervision for problem resolution when necessary.
    • Attend and actively participate in all department meetings, offering input on departmental or clinical matters.

    • Ensure timely and appropriate documentation in the patient medical record. Attend staff meetings, group and individual supervision sessions, and relevant in-services.
    • Engage in projects, tasks, and opportunities to enhance skills and expand competencies.
    • Comply with organizational policies and procedures.
    • Adhere to all established practice standards.
    • Participate in on-call rotation if applicable.
    • Train and supervise students/interns in the department as requested.
    • Promote the organization's Mission and goals.
    • Undertake any other assigned duties.

    Rogers Operating System (ROS)

    • Acquire knowledge of the ROS and MDI process.
    • Participate in the ROS program regularly.
    • Develop and implement ROS projects to drive improvement.
    • Apply the ROS model to departmental activities.
    • Set measurable goals and demonstrate achievement.
    • Engage in daily huddles with assigned team(s).
    • Demonstrate comprehension of Joint Commission, State, and other regulatory compliance regulations.
    • Understand and apply standards relevant to outpatient behavioral health programs.
    • Enhance awareness of regulations and educate others on compliance.

    Ensure patient safety and maintain facility security

    • Take appropriate actions to safeguard patient safety.
    • Report any safety concerns promptly to the treatment team and document all incidents.
    • Ensure patients are in appropriate areas at all times.
    • Promptly report safety hazards to the Director of Operations and/or Operations Manager.
    • Document safety incidents via the designated portal.
    • Adhere to all safety guidelines outlined by the System.
    • Follow emergency procedures as required.

    Additional Job Description:

    Education/Training Requirements:

    • Hold a Master's Degree in Social Work, or a related field, from an accredited Social Work Professional Counseling or Family Therapy school, with preference for experience in healthcare and psychiatric settings.
    • Currently pursuing licensure as a Licensed Professional Counselor (LPC-IT), Marriage and Family Therapist Training License (MFT-TL), Social Worker Training Certificate (SWTC), Advance Practice Social Work (APSW), or equivalent based on state requirements.
    • A SAC-IT, SAC, or CSAC certification is mandatory in WI. A substance abuse specialty authorization relevant to the hiring state is essential.
    • Possess working knowledge of therapeutic intervention and techniques across all age groups, as well as understanding human growth and behavior dynamics.
    • As indicated by the program description, this role might encompass cognitive behavioral therapies and social services responsibilities.
    • American Red Cross Professional Rescue certification is necessary within 30 days of hire, with bi-annual recertification.
    • Formal training in managing aggressive patients (NCI) is compulsory within 60 days of hire, with annual re-certification.
    • Attend all mandatory and relevant in-services to meet educational obligations concerning the program/unit license regulatory requirements.

    The System comprises Rogers Behavioral Health Foundation, supporting patient care, programs, and research, and Rogers InHealth, working to combat the stigma around mental health difficulties.
