As part of a project funding from the Ministry of Social Affairs and Health, Caritas-Altenhilfe is breaking new ground and expanding its portfolio with this innovative offer:

For our innovative short-term care in the new Zoffingen house in Konstanz, located on the shores of Lake Constance, we are looking for a Palliative Care Specialist (m/f/d) as part of the QM team with regular working hours, starting immediately or by agreement with a workload of 30%.

The short-term care with 15 beds is a model project with a dual specialization in the care and support of people in a palliative situation and the care of people after outpatient or inpatient treatments. The project is supported by project funding from the Ministry of Social Affairs and Health.

Your tasks

Care for palliative clients in this special 15-bed department

Assessment, planning, coordination and evaluation of palliative care and support

Initiation and support of adjustments in the care process

Close support of patients and their families in the final phase of life

Coaching, instruction and support of nursing staff in palliative care situations
