    Main responsibilities of a Home Health Registered Nurse (RN):

    • Provide in-home care to patients recovering from illnesses, injuries, or chronic conditions post-discharge
    • Collaborate with physicians and a multidisciplinary team to ensure patient well-being
    • Conduct patient assessments, document progress, and communicate changes to the physician
    • Educate patients on their care plan, medications, therapy, diet, and exercise
    • Train family members on the patient's care plan, medications, and home safety
    • Coordinate goals achievement and discharge planning

    Requirements for Home Health Registered Nurses (RN):

    • Graduate of an accredited Registered Nurse (RN) program
    • Active, unrestricted RN license in South Carolina or compact multi-state license
    • Minimum of one year nursing experience, preferably in home healthcare, Med/Surg, or Critical Care
