Requirements:

Bachelor's Degree in Social Science or related fields
Proficiency in Microsoft Office and computer skills
Strong Oral and Written Communication Skills
Flexible to work Non-traditional hours
State of Alabama Adult Case Management Training or willingness to attend
Familiarity with Community Lay-out
Knowledge of Community Agencies and Resources
Valid driver's license with a clean record for 5 years
Completion of required trainings including Homeless Management Information System (HMIS)

Responsibilities Overview:

- Attend Continuum of Care (CoC) meetings as the main contact

- Collaborate closely with supervisor and community stakeholders to support clients

- Conduct assessments and monitor client progress in their environments

- Act as the key case manager for various housing programs and caseloads

- Ensure compliance with HUD standards and maintain accurate documentation

- Provide training sessions and support in basic living skills

- Assist clients with appointments and scheduling

- Respond to client crises and emergencies as needed

- Input and manage data in relevant systems
