Position Summary:


As Resource Case Coordinator with the Ministry of Caring you would be responsible to provide case management services to persons with HIV/AIDS residing at House of Joseph II.

In cooperation with the facility's Registered Nurse, you would develop and implement a service plan to address the social, physical, financial, and material needs of each person and ensure that deserved and needed services are found through linkages with existing support services or referrals to other agencies.


Essential Functions:


  • Help residents make informed decisions by acting as their advocate regarding their clinical status and treatment options.
  • Provide an initial comprehensive assessment of each resident's needs and personal support systems.
  • Develop a comprehensive and individualized service plan for each resident in collaboration with the Director and Registered Nurse.
  • Monitor the service plan regularly to determine efficiencies and the need for revision.
  • Accurately enter and reconcile client data in the State of Delaware's global database and run accurate reports from that system. (CMIS)
  • Communicate to local agencies, nonprofit of upcoming vacancies and the status of the programs waiting list.
  • Report on each resident's progress with their care plan at monthly administrative team meetings.
  • Involve residents in planning and implementing indoor and outdoor monthly social activities.
  • Other duties needed to help drive our vision, and fulfill our mission, of "The Poor should never be treated poorly".

Education/Experience:


  • High school diploma or GED a must, bachelor's degree in related healthcare field preferred
  • Proven working experience in case management, including as a nurse, medical mental health care, or previous related position.

  • Strong verbal and written communication skills
  • Able to effectively manage deadlines.
  • Works well under pressure.
  • Strong organizational skills and detail oriented, with good time management skills.

Job Type:
Part-time


Pay:
$ $23.41 per hour


Expected hours:

Schedule:

  • 8 hour shift
  • Evening shift
  • Monday to Friday
  • Weekends as needed

Ability to Relocate:

  • Wilmington, DE: Relocate before starting work (required)
