- Makes home visits for physical and emotional assessment of patient/family units. Contacts patient/family by phone between visits.
- Develops and maintains a written Hospice Care Plan for each patient and updates twice monthly or more often if needed.
- Maintains complete, accurate and up-to-date medical records and other records pertaining to the provision of care. Documents all contacts and has documentation in office per policy.
- Participates in weekly patient care conferences.
- Communicates to other team members the current status of the patient/family unit.
- Collaborates with Interdisciplinary Group members to ensure the best care for the patient/family according to the Hospice concept.
- Develops Hospice Aide plan of care based on patient's personal care needs.
- Assists in creating and maintaining a climate that promotes growth and harmonious relationships within the Hospice staff.
- Promotes a positive image of the Hospice concept throughout the community.
- Competently performs all basic nursing skills
- Determines the patient is hospice appropriate, documents to the patient's terminality, and establishes and maintains the care plan for medically necessary assessments and services. Attending Physician or Hospice physician, in all circumstances in which the level of care is to be assigned as Short-term Inpatient Care or Continuous Care, and includes all actions required in Covenant's policies and procedures regarding the placement or removal of a patient from those categories of care.
- Performs other duties as may be assigned by the Clinical Manager or Branch Office Manager.
- Required to comply with all Covenant Hospice Policies, Procedures and Personnel Policies, including those specifically addressing disaster preparedness and response.

The Interdisciplinary Group Weekend RN is a professional nurse specially trained in the Hospice concept to provide on-call service for the personal, physical, emotional and social needs of the patient family unit.
The weekend RN will be responsible for evaluating and responding to the patient's/caregiver's needs via telephone contact, home visits, and/or facility visits for assessment, symptom management, treatment, teaching and coordination of care.
A minimum of one year of clinical nursing experience preferred. Experience must be in Hospice or in a medical/surgical setting, a hospital, oncology or geriatric facility.Reliable transportation, valid state of Florida or Alabama driver's license, as applicable depending on assignment, and current automobile liability insurance and insurability under the company liability insurance are required.
Responsible for providing on-call services for personal, physical and emotional needs of the patient/family unit.Normally, such lifting will include, but is not limited to, assisting with transfer/repositioning of the patient either in the home or nursing facility.
This position is also mentally demanding because of the continual need of assessing and evaluating certain clinical aspects of the patient's care.
This position requires considerable time traveling in a motor vehicle in connection with performing the essential functions of the position.
