Job description

The Patient Advocate works primarily during clinic sessions providing support with patient follow-up, including case management and advocacy efforts. The Patient Advocate provides social service and community referrals for patients as well as identifying and proactively handling other patient inquiries and needs. This position requires a compassionate, culturally sensitive and detail-oriented person who can work independently and follow through accordingly, and function well within a small, close-knit team.

Essential Functions

  • Assists with the patient registration/check-in/out process and collection of donations during evening clinics.
  • Works directly with patients (in-person or by phone) to assess needs and eligibility to determine the best resource(s) for medical, specialist & other community referrals.
  • Refer patients to appropriate programs and services and assist patients with the enrollment / sign-up process as needed
  • Uses AtheanaNet, VHC's electronic medical records system, to work assigned referrals, navigate patients' electronic medical charts for details and document all patient interactions in electronic format (by creating/updating Patient Cases in AthenaNet)
  • Meets with parents of pediatric patients to determine eligibility for CHIP/Medicaid program and provides referral information to Medicaid/CHIP
  • Manages the Chronic Disease Management (CDM) enrollment process and audits the CDM schedule and patient charts for recent labs to ensure CDM clinic resources are optimized
  • Assists volunteers with any clinic process-related questions during clinic sessions
  • Carefully tracks and prepares reports regarding referrals, patient advocacy activities, and other appropriate information using the tools and spreadsheets provided by Patient Services Director
  • Maintains the resource directory and distributes referral information as needed
  • Participates in community outreach and represent VHC at resources fairs
  • Regularly meet and report to the Patient Services Director regarding patient care needs and any concerns as well as status updates on patient case management
  • Other duties as assigned or as requested by the Patient Services Director, Charge Nurse, or other senior VHC Staff

Knowledge, Skills, and Abilities:

  • Must be Bi-Lingual in English/Spanish (written and spoken)
  • Excellent communication, interpersonal, and customer service skills
  • Strong computer skills including the ability to learn & navigate an EMR system (AthenaNet)
  • Self-Directed / Detail-oriented / Excellent follow-through with minimal supervision
  • The ability to work in a team-based environment in a manner that fosters cooperation

Education and Experience

  • Bachelor's Degree
  • A minimum of one year experience in making referrals and case management
  • Study in Social Work or a related field preferred
  • 1-2 years as a volunteer in agency helpful
