- Conducts comprehensive assessment of patient's needs, develops treatment plan, and implements occupational therapy care plan at patients' homes or facilities.
- Maintains, updates patient records by completing required documentation and ensuring compliance with requirements.
- Monitors patient's progress, revises treatment plan and discharge plan as needed.
- Travels to various environments to deliver care and assist patients with daily activities.
- Provides care in accordance with Agency standards, including using medical equipment.
- Counsels, educates patients/caregivers, and evaluates equipment needs.
- Manages caseload, evaluates appropriateness of services, and participates in professional growth activities.
- Participates in meetings and special projects as assigned.

Occupational therapist needed to provide services to VNS Home Care patients by conducting assessments and implementing care plans. Works under supervision.
Compensation:$85, $106,300.00 Annual
What We Provide
What You Will Do
Minimum one-year occupational therapy experience or completion of probationary period as a VNS Health Rehab Intern
Licensure as an Occupational Therapist in New York State
Valid driver's license required for areas outside Manhattan
Pay Range per Visit:
If applying to per diem version, hiring range is $70- $75 per visit.
