Looking for Registered Dietitian to join our Skilled Nursing Home in the Queens area. - MUST BE LICENSED OR RD EXAM ELIGIBLE
Accurately assess patient's nutritional status, identify nutritional risk, develop plan of care and evaluate outcomes.
Develop a written dietary plan of care that identifies the dietary problems/needs of the resident and the goals to be accomplished for each dietary problem/need identified.
Participate in meal rounds to monitor intake and determine/address any issues related to food service and nutritional care.
Participate in comprehensive care plan meetings with interdisciplinary team.
Complete MDS accurately and in a timely manner.
Review monthly weights and identify significant weight change and place appropriate interventions and documentation.
Periodically assess adequacy of nutritional care provided to residents with feeding tubes and pressure ulcers.

Job Type: Part-time position available

Nutritional services are carefully monitored by our dietitians and special consideration is given to meet each resident's nutritional needs.
