Description
We are an Assisted Living Facility located in Staten Island seeks an experienced Case Manager.Some of the case manager responsibilities include; identifying, addressing, and properly documenting efforts to address the needs of our residents.
Responsibilities
- Coordinate and provide care that is safe, timely, effective, efficient, equitable, and residentcentered
- Help residents make informed decisions by acting as their advocate regarding their clinical status and treatment options
- Develop effective working relations and cooperate with professional entities
- Take the extra mile and interact with residents to keep track of their progress and to ensure satisfaction
- Record cases information, complete accurately all necessary forms and produce statistical reports
- Assess and address motivational and psychosocial issues
- Adhere to professional standards as outlined by protocols, rules and regulations
Requirements:
- Bachelor's degree in social work or related field.
- Minimum of 1 year case management experience.
- Excellent verbal and writing communication skills.
- Compassionate and understanding of resident needs.
- Comfortable using computers.
