Qualifications

  • Bachelor's Degree in Human Service/Counseling related field or equivalent experience in full time case management
  • Experience in the nonprofit field; human service field
  • Knowledge of crisis intervention in the company of persons with behavioral health issues
  • Ability to network with other agencies and their representatives
  • Budgeting acumen
  • Ability to function with diverse populations
  • Possession of a valid driver's license
  • Flexibility

Basic Functions

  • Working with program participants experiencing homelessness to obtain housing and provide case management that includes supportive services that assist with life skills
  • Providing housing counseling and referrals to community services

Typical Activities

  • Demonstrate in day to day behavior the values inherent in the mission statement of the Catherine McAuley Center.
  • Outreach collaboratively with peer agencies
  • Prepare reports
  • Attend all McAuley case management meetings, provide community outreach and represent the Center at meetings, events, conferences, etc.
  • Work as a team player.
  • Fully participate in all of the Center's staff meetings, major client events and fundraising events, as needed.
  • Maintain required documentation, including case notes
  • Seek out safe, suitable housing for clients
  • Complete the necessary initial paperwork with each client setting initial goals and completing housing regulation forms. assist the clients in setting goals monthly that work toward self sufficiency, making sure the clients work toward meeting the goals and objectives of the housing program as well as their own personally set goals. Inspect the apartments for safety and cleanliness purposes as needed, at least once a month.
  • Meet with each new client at least weekly for their personal, individual life development needs and review of their most current goal plan. Assist clients with the significance of personal financial management and basic budgeting skills.
  • Work with clients so that they grasp the major tenants of the program and their continuing responsibilities.
  • Supervise the planning and facilitating of the group meetings, education opportunities, house inspections, life skill development sessions, group laundry events, grocery shopping events and activities.
  • Coordinate clients' transportation to all appointments necessary to their health and well being.
  • Position requires being "on call" after hours and on the weekend for one week every eight weeks as needed for emergency response.

From $20.00 per hour
