Job Summary

Responsibilities

  • Coordinate with colleagues within the office and field to assess, plan, implement, or evaluate patient care plans.
  • Ensure all orders are received and processed timely for patient delivery.
  • Communicate effectively with hospice team regarding patient needs and facilitate multiple care aspects.
  • Participate in hospice IDT meetings as part of an interdisciplinary team of Nurses, Physicians, Social Workers and Chaplain.
  • Record information with accuracy and produce reports as needed.
  • Provide compassionate and caring interaction with patients/families to ensure satisfaction.
  • Promote high quality and costeffective interventions and outcomes.
  • Maintain accurate records of patient care and discuss observations with the supervisor.
  • Adhere to professional standards as outlined by protocols, rules and regulations.

Qualifications

  • Currently licensed as a Vocational Nurse in state of California
  • Previous experience as an LVN case manager is preferred
  • Technical skills needed for Microsoft, Excel, Google Docs and EMR system
  • Excellent interpersonal and problem solving skills
  • Strong organizational and time management skills
  • Ability to multitask and work as part of an interdisciplinary team
  • Compassionate and able to discuss difficult topics related to endoflife
  • Determined and interested to learn and grow with the hospice
