- Performs a basic physical assessment
- Understands signs and symptoms and data related to the patient's presenting problems and potential problems.
- Reassess and evaluates patient needs each visit as indicated.
- Assesses patient and environmental safety continuously.
- Notifies physician, NP and transplant coordinator of pertinent changes in patient condition.
- Recognizes and appropriately reports basic abnormal data and subtle changes in patient condition.
- Collaborates with other health team members to collect and share pertinent data regarding patient's psychological and physiological condition.
- Assesses need for referral to other health care professionals.
- Prepares, revises, and coordinates interdisciplinary plan of care for each patient in collaboration with other transplant team members and disciplines.
- Includes patient and family in developing goals that are mutually agreed upon.
- Assures plan of care is consistent with therapy prescribed by the medical practitioner and standards of care.
- Communicates formulated plan of care, as appropriate, to all levels of associates involved in the patients care.
- Reviews and modifies plan of care based on changing patient needs.
- Demonstrates ability to establish priorities of care.
- Updates computerized patient profiles each visit and incorporates changes in plan of care
- Implements nursing actions, either by direct care or delegation of responsibility, based upon patient's plan of care and skill level of personnel.
- Implements plan of nursing care in collaboration with the patient, significant others, and other health care team members.
- Provides ongoing individualized holistic patient and family education based on needs and plan of care.
- Demonstrates critical thinking in review of physician orders and accountability for appropriate implementation of physician orders.
- Demonstrates responsibility for medication administration as allowed by licensure and institution policy.
- Responds to emergency and other situations requiring immediate attention in a controlled precise and skilled manner.
- Makes appropriate referrals to the other health care team members in a timely manner.
- Implements safety interventions to maintain patient / family/staff safety.
- Reports actual and potential patient safety issues.
- Implements measures to prevent contamination and / or transmission of disease
- Utilizes appropriate protective devices and equipment to prevent injury.
- Evaluates the effectiveness of nursing interventions.
- Involves patient and other health team members in the evaluation process when appropriate.
- Documents timely and according to policy and procedure.
- Completes forms accurately and completely.
- Documents patient / family teaching and response appropriately
- Participates in 75% of staff meetings
- Supports self and peers in professional growth and development utilizing resources and opportunities within and outside of The Christ Hospital
- Completes all educational requirements to maintain competency related to specific population of patients and / or regulatory agencies (Online education 100% on time completion). Submits required tests and paperwork in a timely manner without management intervention.
- Participates in self and peer review activities to include the positive recognition of peers and staff on a regular basis
- Evaluates and participates in performance improvement initiatives
- Participation in community service activities is encouraged
- Participation in conferences and educational workshops in relation to current field is encouraged.
- Manages workload effectively.

- Supports effective use of staff and supplies.
- Places charges accurately and timely as indicated.
- Obtains signed ABN from patient as appropriate and scans into EPIC
