We are in search of an Occupational Therapist to become a part of our team at Concord Post Acute.

Full-Time Position
Rate: $60 to $68 per hour

Duties and Responsibilities:

Create effective treatment strategies and secure approval for services from the referring physician
Administer care to patients based on the physician's treatment plan
Support the nursing department by educating Restorative Aides
Supervise Occupational Therapy Assistants in providing direct patient care and related activities, in compliance with the state practice act
Keep the supervisor and other healthcare professionals informed about patient progress, issues, and strategies
Participate in various meetings including Patient Care Conferences, Utilization Review sessions, and Rehabilitation Conferences when necessary
Contribute to the in-service training initiatives for other facility staff
Document evaluations, daily treatment notes, weekly progress notes, and recertification details as mandated by the Occupational Therapy Board State Practice Act and relevant regulations
Update treatment plans in accordance with policies and procedures
Provide guidance to patients' families or nursing staff on maintenance programs and caregiver training for the post-therapy phase
Assist in discharge planning
Suggest and support the procurement of essential durable medical equipment to promote patient mobility and independence
Report any equipment malfunctions within the department for timely maintenance

Requirements:

Valid Occupational Therapy License
