Health Link Home Health and Hospice is a well-known provider of comprehensive home health services in the greater San Francisco Bay Area. With almost two decades of commitment to delivering top-notch care and enhancing healthcare outcomes, we are now on the lookout for dedicated individuals who are passionate about joining our team. We are currently seeking a caring, organized, and team-focused individual to fill the position of Full-Time, Part-Time, or Per Diem Occupational Therapist (OT) for the Oakland-Richmond area, as well as covering Walnut Creek-Brentwood locations.

In this crucial role, you will collaborate closely with our Director of Nursing, clinical team, and front office staff to ensure seamless patient care and provide assistance to our exceptional healthcare professionals. Your contributions will play a key role in upholding our reputation for excellence and elevating the overall patient experience.

Responsibilities:

Provide occupational therapy services as per the care plan and in coordination with the Interdisciplinary Team
Adhere to the care plan, offer treatment, assess patient response, and report any changes to the Case Manager and/or Director of Nursing
Complete online charting on Kinnser promptly using company-provided devices
Ensure compliance with all company policies and procedures

Requirements:

Active OT License in California
Valid CA Driver's License
Ability to travel within a 15 to 25-mile radius regularly
