About us
Health Projects Center is a 501(c)3 non-profit corporation established in 1980. We provide exceptional community-based programs that support elders to live and age with dignity at home. We deliver culturally appropriate programs in both English and Spanish for older adults, family caregivers and health workers.

Health Projects Center serves Monterey, San Benito, and Santa Cruz Counties.

We run the Del Mar Caregiver Resource Center, Multipurpose Senior Services Program, and the Central Coast Area Health Education Center.

Our programs support over 1,300 families annually to live and age at home at a fraction of the cost of an institution.

The Associate Family Consultant role at Health Projects Center plays a pivotal part in our mission to support elders and their families in living and aging with dignity at home.

This position serves as a crucial link between our programs and the community, providing essential support to Family Consultants, maintaining vital client relationships, and ensuring the seamless delivery of our services.

This position assists in client enrollment, maintaining educational resources, and facilitating caregiver education and training.

Additionally, their involvement in program reporting, content development for caregiver newsletters, and participation in outreach events further solidifies our commitment to enhancing the lives of older adults and their caregivers through accessible, culturally appropriate support services.


Essential Functions:


  • Supports Family Consultants (FC) with casework activities:
  • Prepares and maintains client enrollment packets.
  • Mails out forms and other correspondence to caregivers as directed by the FC.
  • Follows up with caregivers on behalf of FC with resource information. Includes assisted referrals connecting to community service providers, researching service options, and mailing out resource information.
  • Follows up with caregivers regarding use and satisfaction of vouchered services.
  • Provides monitoring contacts to caregivers as directed by the FC.
  • Meticulously documenting all client activities in the client's electronic record.
  • Identifies and maintains caregiver educational resource materials.
  • Supports caregiver education and training activities, including registering participants for caregiver education workshops.
  • Prepares program reports for state and local funders.
  • Develops content for caregiver newsletter.
  • Implements client surveys.
  • Assists with Caregiver University Conferences and outreach events as needed.

Qualifications:


  • Relevant work experience

Preferred Qualifications:


  • Bilingual (English/Spanish)
  • Bachelor's degree in relevant field
  • Capacity to effectively manage a hybrid work environment.
  • Ability to navigate local community referral systems and utilize available resources.
  • Strong organizational skills coupled with effective time management.
  • Demonstrated ability to work independently.
  • A positive approach to work.
  • Capacity to work with databases and electronic data systems effectively.
  • Ability to support and collaborate with diverse clients and staff.

Conditions of Employment

  • Possession of a valid California driver's license and access to an insured and reliable vehicle.
  • Must pass a criminal background check.
  • COVID19 vaccination required.
  • Non-Exempt Status

Compensation and Benefits

  • Starting salary is $23.00$26.00 per hour depending upon skills and experience. We pride ourselves in maintaining a flexible and friendly work environment.

We provide comprehensive benefits including:

paid vacation (15 days); paid sick leave (80 hours); company paid medical, dental, vision, chiropractor/acupuncture, and life insurance; travel reimbursement; educational/fitness allowance; and pre-tax retirement plan with 6% of salary contributed by employer.
