Responsibilities:


  • Provide information, education, and resources on all GSIL services and other relevant community resources to consumers, caregivers or family members, providers, and other interested parties.
  • Facilitate and assist in the navigation of resources in response to the customers' inquiries, such as Service Link and 21
  • Provide all daytoday functions for Information and Referral Services. Follow the policies and procedures that are established to provide high quality and responsive service from initial request and keep resource information updated.
  • Consult with staff on requests for all GSIL services and refer consumers accordingly.
  • Act as the primary agency resource for GSIL services. Must maintain active / current knowledge base of all GSIL services and processes for referrals.
  • Maintain a positive working relationship with community organizations to facilitate, promote collaboration and network in partnership with the Director Community Partnerships.
  • Disseminate, as appropriate and /or requested, GSIL service materials.
  • Assure a seamless referral process to all GSIL services through effective communication, strong customer service approach, and coordination.
  • Serve as a resource on all GSIL services for GSIL staff.
  • Assist the designated Director with evaluating the effectiveness of GSIL referral process for the designated service provision.
  • Maintain accurate documentation of the I & R calls in accordance with GSIL policy and procedures. Provide ongoing feedback to designated Directors as needed to improve systems and communication.
  • Conduct other duties as assigned that will increase options for persons with disabilities to live independently in the community.

Other Responsibilities:


  • Maintain a positive upbeat approach/attitude to foster customer satisfaction.
  • Complete assigned LTS administrative tasks.
  • Participate in general GSIL events and activities.
  • Maintain professional knowledge/skills by reviewing professional literature and attending trainings/workshops.
  • Support and uphold workplace regulations and safety practices.
  • Share newsworthy information appropriately throughout the organization.
  • Conduct other duties as assigned that will increase options for persons with disabilities to live independently in their communities.

Qualifications:

A minimum of two years experience working with persons with disabilities and/or health care needs. LPN or LNA preferred but not required. Must hold a BA degree in related industry. Must have excellent oral and written communications and computer skills required. Must have a commitment to facilitating and maximizing independence for persons with chronic health care needs. Possess excellent customer service skills, assuring timely and effective follow-up and problem resolution.
