Description

You will act as a liaison for our skilled nursing facility and assigned hospitals and designated referral sources for families.

This person will be responsible for building relationships with hospitals, doctors, and case managers, which generate patient referrals and admissions.

You will assess clinical needs of potential / existing residents to ensure the facility has the clinical capability to treat the resident.

• Interacts with community leaders, doctors, social workers, case managers, and discharge planners to maintain and develop referral relationships.
• Work with the Admissions department to meet shortterm and longrange census goals.
• Create, manage and monitor new business development plans
• Communicate with the Administrator to be abreast of policies affecting Admissions.
• Perform an assessment evaluation and a complete indepth physical examination on residents to distinguish between normal and abnormal findings in order to recognize early stages of serious physical, emotional or mental problems.
• Record physical findings, and formulate plans and prognosis based on resident's condition. Report these to the attending physician.
• Order, interpret, and evaluate diagnostic tests to identify and assess resident's clinical problems and health care needs.
• Implement and maintain established policies, procedures, objectives, quality assurance, safety and environmental and infection control. Interpret these to the physician, resident, family members, and public, as appropriate.
