ESSENTIAL FUNCTIONS

Patient Guidance and Education:


  • Monitor goals and act as a supportive resource for patients, helping them understand and navigate their treatment plans, in the context of lifestyle and functional medicine.
  • Provide evidencebased education on lifestyle factors, nutrition, stress management, and other relevant topics to empower patients in taking control of their health.
  • Developing learning and educational content based on coaching principles and collaboration with medical professionals for use with patients in care plan development and coaching.

Individualized Health Coaching:


  • Conduct independent inperson or virtual oneonone and group health coaching sessions to collaborate with patients to set goals and personalize healthcare teams' plans for achieving optimal health.
  • Collaborate with healthcare providers to integrate lifestyle and functional medicine interventions into patients' treatment plans.
  • Research care pathways that are evidencebased to support functional, integrative, and lifestyle related care plans for patient care, with collaboration with health professionals.

Care Coordination:


  • Work closely with various health care professionals including physicians, nurse practitioners, and registered dietitians to coordinate and facilitate seamless patient care.
  • Present patient information at the daily or weekly huddles.
  • Assist in implementing care plans that align with our holistic approach and patient's individual health needs.
  • Support patients with internal and external referrals and scheduling of services in the wellness and fitness center.

Patient Advocacy:


  • Advocate for patients' needs and preferences within the healthcare system.
  • Assist patients in navigating insurance, referrals, and other administrative aspects of their healthcare.

Support Group Facilitation:


  • Assist in the research and creation of content for online and group education.
  • Participate in development and deliver group education sessions.
  • Organize and lead support groups to foster a sense of community and mutual support among patients with similar health challenges.
  • Participate in community outreach projects and support programing for organization.

JOB KNOWLEDGE, SKILLS, ABILITIES


This Health Coach and Care Navigator must possess the following knowledge, skills, and abilities to demonstrate they can perform the essential functions of the job, along with the physical requirements, with or without reasonable accommodation.


  • Strong understanding of primary care, functional, integrative and lifestyle medicine concepts.
  • Excellent communication and interpersonal skills.
  • Empathetic and compassionate approach to patient care.
  • Strong familiarity with Electronic Health Records systems.

EXPERIENCE/EDUCATION REQUIRED

  • Bachelor's degree in Health Coaching, Nutrition, Nursing, or a related field.
  • Currently certified in health coaching or pursuing accreditation.
  • Current BLS with AED certificate from the American Heart Association or similar organization.
  • Registered Dietitian, Nursing, or applicable professional preferred.
  • Experience with lifestyle and integrative medicine practice (preferred)
  • Previous experience in health coaching or care navigation is preferred.
  • Bilingual (Spanish) Preferred
