SUMMARY:
The C.E. Specialist performs various duties relative to the C.E. process and navigation. Primarily this position is responsible for the initial and ongoing case management of C.E.
eligible households to ensure that there is a streamlined procedure for assessing and being matched for housing services through our Homeless Management Information System (HMIS).

ESSENTIAL RESPONSIBILITIES AND DUTIES:

Operating as the initial contact for the Coordinated Entry (C.E.) System
Completing intake packets and conducting C.E. assessments
Notifying households of Eligibility, Rights, and Responsibilities, and next steps
Collecting and uploading all documents available at assessment
Assisting households in obtaining necessary documentation required for housing.
Securing additional financial assistance if needed, providing transportation when allowable, accompanying households to potential housing options, etc.
Participating in case conferences
Responding to the C.E. requests by THHI.
Complies with all applicable training requirements.
Complies with all company safety, personnel and operations policies and procedures.
Complies with work schedule to ensure effective operations of Agency programs.
Contributes positively as a member of a productive and cooperative team.
Performs other duties as necessary to fulfill the Agency's Mission

REQUIREMENTS:

Bachelor's degree in social work, human services, psychology, or other related fields from an accredited institution required.
Minimum of two years of case management experience
Knowledge of homeless and community services and advocacy and mediation skills
Demonstrated sensitivity and ability in working with the diversity of the agency's population, including but not limited to, clients, staff, community partners, and board of directors.
Excellent interpersonal and written communication skills.
Ability to walk, stand, climb stairs, kneel, bend, reach, and manipulate objects; move materials up to 5 pounds on a regular basis; infrequently require moving materials weighing up to 20 pounds.
Some local travel required. Must have a reliable car, active auto insurance, and valid Florida driver's license.
Successful completion of a Level 2 criminal background check, motor vehicle records check, and drug screening.
