    Overview


    The Crisis Counselor–Licensed is a professional who holds either a RCSW, LCSW, LMHC or psychologist who always adheres to the highest standards of evidence-based practice, ethics, and professionalism. Provides evidence-informed crisis intervention, suicide prevention, behavioral health psychosocial assessments, screening for admission, information & referral, treatment plans, and brief supportive counseling to patients who are in emotional distress, and/or have substance abuse problems for children, adolescents, and adults. Well-established to work with a team to provide patients with education, support, and counseling in understanding the emotional, psychological, and behavioral impact of mental health, substance use, and addiction on the patient and family. The work involves screening patients in collaboration with other medical staff through integration. This position works in acute settings that address crisis mental health and substance use care between crisis settings as well as in an outreach setting with a multidisciplinary team.

    Responsibilities



    Provide Medically necessary Crisis Assessment, Treatment, and Follow-up Care

    • Provide evidence-based assessment, diagnosis according to DSM-V consultation, and brief intervention for psychological/psychiatric/substance use problems and disorders for children and adults.
    • Provide comprehensive evidence-based triage, assessment, diagnosis, and treatment of behavioral health and substance use disorder patients for children and adults.
    • Recommend and execute appropriate behavioral health and substance use referrals, including involuntary or voluntary psych/substance use diagnosis
    • Assessments and diagnosis should comply with State, and Federal regulations, as well as Health Care District policy and procedures.
    • Work closely with patients to engage them in the decision-making process and help them understand the benefits of assessment and accessing the appropriate level of care as indicated.
    • Develop appropriate intervention and recovery plans for individuals, families, and groups.
    • Provide clinical consultation to clinical and non-clinical team members, referral resources, mentors, family members, and others to help support patients' recovery.
    • Provide brief therapy interventions appropriate for crisis care.
    • Develop treatment plans appropriate to crisis care with patients to clarify goals and objectives during treatment.
    • Model and support health and wellness activities, and healthy coping skills and boundaries for patients.
    • Monitor and participate in outreach to patients following discharge to ensure adherence to treatment recommendations and address barriers to adherence.
    • Conduct practice measures with all patients receiving substance use services under the behavioral health and substance useprogram. Also, participate in program model development and evaluation, and collect and compile data as required by the Behavioral Health Director.
    • Maintain working knowledge of other drug and alcohol treatment agencies, programs, and a list of resources that can be shared with patients.
    • Provide crisis intervention, counseling, information, and/or referrals to patients reaching out using a phone call.
    • Requires working in a 24x7 model and must be available to work night and weekend shifts.
    • Maybe cross-trained/scheduled to work in the Clinic, on the phone, and/or on a mobile crisis team with law enforcement.
    • Acts in a manner that protects the integrity of the Health Care District's Mission.
    • Emergency duty may be required of the incumbent including working in Red Cross shelters or performing other emergency duties including, but not limited to, responses to threats or disasters, man-made or natural. Additionally, incumbents are required to perform emergency response and management duties for Palm Beach County as required.

    Collaborate with Appropriate Systems Involved withPatient Care

    • Work collaboratively with community-based partners regarding incoming and outgoing referrals to provide patient-centered coordinated care.
    • Establish and maintain a current listing of community resources to meet the needs of the target population.
    • Develop and maintain collaborative working relationships with community providers to perform case management services as needed.

    Serve on Interdisciplinary Team, Collaborate to EnsureAll Patients Receive Appropriate Care

    • Work with a multidisciplinary psychiatric and substance use team mixed with clinical and non-clinical team members collaboratively to provide excellent patient-centered care.
    • Consult with supervisor and/or psychiatrist and medical team as needed.

    Complete all Administrative Tasks/ Functions Appropriate to the Role to Support Operations

    • Show competence in utilizing the EHR and other IT solutions regarding the patient's episode of care.
    • Maintain confidentiality of EHR records relating to patients' treatment as per HIPPA laws.
    • Submit quarterly reports of Peer Review Audits to the Behavioral Health Director. Participate in bi-weekly peer supervision and monthly provider meetings.
    • Participate in program model development and evaluation, and collect and compile data as required by the Behavioral Health Services Director.
    • Participate in all assigned meetings, staff development, and training as required.
    • Remain in compliance with internal and external policies, procedures, regulations, and requirements, State Licensing standards, County Monitoring requirements, and managed care regulations.

    Qualifications


    Education:

    • Master's degree in Psychology, Social Work, Mental Health Counseling, or related field required.

    Experience:

    • Three (3) years of professional experience in treating behavioral health, alcohol, and substance abuse in a healthcare setting is required.

    Certification:

    • N/A

    Licensure:

    • A current and valid State of Florida License in Mental Health Counseling, Clinical Social Worker or Marriage and Family Clinical Psychology is required.
    • Valid Florida Driver's license is required.
