    Position Purpose

    The Program Specialist is responsible for providing information and referral services to HIV+ individuals, families, significant others and the general public; determine eligibility, and complete intakes. The Program Specialist is responsible for data entry of intakes and eligibility procedures to enroll clients into Food Bank and Housing Navigation services, in addition to other data-related tasks assigned by supervisor. The Program Specialist will be responsible for the eligibility updates and intakes of interagency clients with the Food Bank.

    **Scope of Responsibilities**

    ****Position Responsibilities****

    Assist in determining client eligibility for the various Vivent Health services specifically Food and Nutrition Services - and other community resources.

    Stay up to date on agency eligibility policy, proper eligibility documentation requirements, and use of agency forms.

    Collect eligibility documents from clients that fit current agency requirements, and assist clients in obtaining or providing such documents.

    Conduct phone and in-person screenings to assess potential clients for services, as needed.

    Conduct agency intakes per agency policy, as needed.

    Conduct eligibility updates of current clients per agency policy, as needed.

    Maintain Housing Navigation client hard files, as specified by Housing program guidelines.

    Maintain Food and Nutrition client hard files, as specified by FNS program guidelines.

    Maintain client record documentation in Provide Enterprise according to departmental standards.

    Enter and scan sensitive client data into Provide Enterprise, including progress logs, units of service, outcome data collection and reporting, and eligibility documentation.

    Enter client data into ARIES and other required databases, per current policy and procedure.

    Meet deadlines for time sheets, client documentation, and required grant documents.

    Assist with Housing Department quality assurance projects, including data quality, tracking, and client hard files.

    Participate in quality improvement activities as assigned.

    Participate in trainings as required by the Austin TGA Standards of Care.

    Promote a positive image of Vivent Health to our clients and the community.

    Attend monthly All-Staff and mandatory organizational meetings.

    Assist in covering reception, when requested by supervisor.

    Attend department meetings for both Food and Nutrition and Housing Services, as requested.

    Perform other duties as assigned.

    Participate in leadership opportunities and committees as needed.

    ****Required Qualifications****

    Knowledge of HIV, mental health, substance abuse, or related field

    Knowledge of diverse populations and community resources

    Skill in working with people with mental health and/or substance use disorders and with diverse populations

    Ability to work comfortably with diverse populations, with sensitivity to issues concerning HIV and all disabilities

    Skill in operating office equipment, such as personal computer, calculator, copier, facsimile machine, and telephone system

    Skill in using various software, including word processing, spreadsheet, and database programs

    Ability to demonstrate good judgment, maintain strict confidentiality standards, and adhere to professional standards as defined by state and federal regulations

    Ability to make sound decisions in accordance with agency policies, procedures, and guidelines

    Ability to take all precautionary steps to ensure the protection and integrity of protected health information

    Ability to prioritize multiple tasks and competing priorities

    Ability to establish and maintain good working relationships with coworkers

    Ability to communicate effectively, both orally and in writing

    Ability to perform routine walking, standing, bending, lifting, and stooping during the course of day

    May be required to operate personal vehicle for department activities and to maintain vehicle liability insurance

    An Influenza vaccine is required of all new employees at the time of employment, and is required thereafter on an annual basis.

    A tuberculin/TB skin test is required of all new employees at the time of employment, and may be required thereafter on an annual basis, depending on position.
