Job Summary:
The Home Health RN is responsible for providing comprehensive nursing care to patients in their homes.

This includes assessing the patient's health status, developing and implementing individualized care plans, and coordinating with other healthcare professionals to ensure continuity of care.

Responsibilities and Duties:

Conduct initial assessments of patients and develop care plans based on their individual needs
Monitor patients' progress and adjust care plans as needed
Administer medications, complete treatments, and provide wound care as necessary
Educate patients and their families on managing their health conditions and promoting wellness
Coordinate with physicians, therapists, and other healthcare providers to ensure comprehensive care
Document patient progress and communicate with other members of the healthcare team

Qualifications and Skills:

Valid RN license in the state of practice
Minimum of 1 year of experience in home health or a related field
Strong clinical assessment and critical thinking skills
Excellent communication and interpersonal abilities
Ability to work independently and prioritize tasks effectively
Proficiency in electronic medical record systems
