- Provide high-quality, compassionate care to homebound patients
- Conduct comprehensive patient assessments and develop individualized care plans
- Administer medications and treatments as prescribed by physicians
- Educate patients and their families on disease management and self-care techniques
- Maintain accurate and timely documentation using Homecare Homebase as a charting system
- Ensure compliance with OASIS documentation with a 24-hour turnaround time
- Achieve weekly productivity goals of 25-30 units
- Potential for on-call and/or weekend shifts
- Valid RN license in the state of Florida
- 1-2 years of experience in home health care as an RN
- Proficiency in Homecare Homebase charting system
- Ability to maintain a 24-hour turnaround time with OASIS documentation
- Strong interpersonal and communication skills
- Reliable transportation and valid driver's license
- Ability to pass medical document review, background check, and drug screen in compliance with the Joint Commission
- Commitment to providing exceptional patient care and support
