- Medication Management: Administer medications and treatments as prescribed, ensuring effective pain and symptom management, while also educating patients and families about medication regimens.
- Comfort and Support: Provide emotional and psychosocial support to patients and their families, helping them navigate the challenges of end-of-life care and offering compassionate guidance.
- Symptom Management: Monitor patients for changes in symptoms and provide timely interventions to maintain their comfort and quality of life.
- Education: Educate patients and families on end-of-life care, pain management, and disease progression, empowering them to make informed decisions.
- Documentation: Maintain accurate and timely electronic medical records (EMRs), documenting patient assessments, care plans, interventions, and outcomes.
- Collaboration: Collaborate with the hospice interdisciplinary team, including physicians, social workers, spiritual counselors, and other healthcare professionals, to provide holistic care.
- Family Support: Offer guidance and support to families in coping with the emotional and practical aspects of end-of-life care.
- Adherence to Regulations: Ensure compliance with all state and federal regulations, as well as hospice policies and procedures.
- Current and valid Registered Nurse (RN) license in state of practice.
- Minimum of 5 years of nursing experience, preferably in hospice or palliative care.
- Excellent communication and interpersonal skills.
- Compassionate and empathetic approach to patient care.
- Ability to work independently and as part of a multidisciplinary team.
- Proficiency in electronic medical records (EMR) systems.
- BLS (Basic Life Support) certification.
