Description

• Assist residents with activities of daily living (ADLs) such as bathing, dressing and grooming. Provides personal care per the residents individualized plan of care. Assist residents with toilet and incontinent care. Makes incontinent checks at least every 2 hours and provides incontinent care as needed.
• Receives and communicates resident(s) status at the beginning and end of each shift and notifies Management of any changes in resident medical conditions/status.
• Provide direct support to residents with mobility issues.
• Implement behavior management techniques when necessary.
• Maintain a clean and organized living environment for residents.
• May help prepare/serve meals and assist residents to/from the dining areas (assists with set up and feeding of meals).
• Make Room Checks every 2 hrs. to verify resident location.
• Keep Resident Rooms neat and clean. Wash and fold resident laundry.
• Maintain a working knowledge of Community Policies & Procedures.
• Completes disciplinary assessments when needed with documentation.
• Communicates effectively with residents, families, visitors, etc.
• Maintains education development to include ongoing training, etc. to maintain compliance with State and Federal Regulations.
• Maintains resident confidentiality and protected health information.

• Previous experience in assisted living, nursing homes, or memory care facilities preferred.
• Knowledge of resident care practices and protocols.
• Ability to assist residents with ADLs and mobility challenges.
• Strong communication skills and the ability to work effectively in a team environment.
• Compassionate and patient demeanor when interacting with residents.
• Standing/walking/stooping/kneeling/crouching, etc.
• Ability to read, write and speak English.
• Ability to lift up to 50 lbs.
