    Title:
    Nurse Practitioner - Advanced Practice Provider

    Location:
    Cedar Rapids

    Salary:
    $105,000-148,500 (Wider range available depending on experience and location)

    Schedule:
    Full-Time, Monday through Friday, 8:00 AM - 5:00 PM

    Role Description:

    The main responsibility of a Nurse Practitioner at Oak Street Health is to provide high-quality primary care to adults on Medicare, ensuring they stay healthy and happy.
    Nurse Practitioners work independently and in conjunction with physicians as required, offering longitudinal care to patients and establishing meaningful relationships.
    In the outpatient setting, Nurse Practitioners collaborate with a healthcare team consisting of physicians, nurses, medical assistants, and support staff to deliver comprehensive care. This allows our providers to focus on primary care, care coordination, and population health within the clinic.
    We also have robust connections with specialists and hospitals for specialized and acute care needs.

    Core Responsibilities:
    - Deliver exceptional primary care
    - Conduct office visits for routine and acute conditions
    - Administer wellness and health risk assessments with detailed documentation
    - Coordinate care with other providers and facilities
    - Lead population health initiatives with a focus on preventive screenings
    - Educate patients on their conditions and treatments
    - Participate in promotional activities
    - Conduct home visits when necessary
    - Other duties as assigned

    This role reports to the Center Medical Director and collaborates closely with operational leadership.

    Required Qualifications:
    - Master of Science in Nursing or equivalent
    - National certification in a relevant specialty
    - Active state Nurse Practitioner license and DEA license
    - US Work Authorization
    - Strong alignment with Oak Street Health's values

    Preferred Qualifications:
    - Experience in primary care such as internal medicine or family medicine
    - Knowledge of HCC documentation and ICD-10 Coding
    - Passion for teamwork and population health initiatives
    - Bilingual proficiency is a plus
