General Summary


Connected Families NH is seeking a Care Coordinator for our FAST (Families and Systems Together) Forward New Hampshire Wraparound practice.

This position will serve youth and families, who are experiencing significant behavioral and mental health challenges, within their homes and communities.

The youths worked with range from 6 -21 in age. The Care Coordinator will become certified in an evidence-based, family-centered practice.

Care Coordinators facilitate a planning process focused on monitoring family outcomes and working with the youth, their family, and community partners to support their success at home, in school, and in their community.

This position will carry a caseload of 8-10 cases.


Pay:
From $27.21 per hour

Expected hours: 40 per week
