Come join a GROWING team We are eagerly seeking an Intake Coordinator Assistant to help navigate our day-to-day family of Home Health Providers and clients.


  • Answering incoming calls to address patients' inquiries and explain the admissions process.
  • Performing administrative duties, including making copies, sending faxes and organizing files.
  • Helping patients schedule their appointments.
  • Functioning as a liaison to referral sources by responding to Company referrals and facilitating the provision of home care services and/or inquiries.
  • Assisting in arranging for uninterrupted continuing medical care of the client at home and additional services needed for effective care of the client.
  • Welcoming, registering, and admitting patients at healthcare facilities.
  • Gathering information about the patient and creating patient records.
  • Making appointments and scheduling procedures for future medical treatments.
  • Answering questions related to services and making referrals when necessary.
