We are currently looking for a Project Connect Coordinator in our Family Support Services Department. This is a part-time role, days of the week flexible, 20 hours/week.

SUMMARY


Under general supervision of a director, is responsible for helping families and others in understanding how to access services and support for individuals with developmental disabilities for Project Connect and housing Assistance Services programs.

Creates and updates flyers promoting agency programs and services.

Help to resolve personal, financial, and environmental difficulties that add to the challenges of providing care for a family member with developmental disabilities.

Enables and empowers families to advocate appropriately.

Works in accordance with ADAPT Community Network (ADAPT) policies, rules and requirements of applicable regulatory agencies and the profession's standards.


ESSENTIAL DUTIES AND RESPONSIBILITIES
The duties and responsibilities of the Project Connect Coordinator will include, but are not limited to the following:

  • Answers the ADAPT Project Connect phone line to assist callers in accessing ADAPT services and support as well as other generic and government related services and supports.
  • Tracks pertinent information, i.e. client demographics, disability characteristics and needs from all callers in Project Connect Database.
  • Manages caseload for Housing Assistance Services program.
  • Places and receives calls to address housing related issues for families with a developmentally disabled family member living in New York City area.
  • Monitors status and makes appropriate recommendations and referrals for callers.
  • Develops collaborative relationships within the community.
  • Provides families and others with resources to be able to meet their needs.
  • Provides interactive presentations for families and professionals and attends Developmental Disabilities Council meetings.
  • Assures confidentiality of information about services.
  • Keeps abreast of changes in field.
  • Represents ADAPT at forums and meetings citywide.
  • Performs other related duties as requested.

QUALIFICATIONS

  • Ability to read, write, speak, and understand English.
  • Good organizational skills and ability to meet deadlines.
  • Good interpersonal skills necessary to interact effectively with coworkers, employees, children and their families, and outside contacts.
  • Proficiency in the use of a computer and knowledge of appropriate software.
  • Due to sensitive nature of the position, must be able to maintain the strictest confidentiality.
  • Ability to work independently and handle multiple tasks.
  • Ability to analyze problems and determine corrective measures.

EDUCATION and/or EXPERIENCE

  • Bachelor's Degree
  • At least one year of related work experience in the field of developmental disabilities

COMPENSATION:
$27.81/hour, 20 hours/week, days flexible
