Provide basic coding functions for simple record types under the close supervision of the coding supervisor/department manager. Ensure ethical and accurate coding in accordance with all regulatory and system requirements and AHIMA Standards of Ethical Coding.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Review all diagnostic and clinical information in the medical record.
  • Complete coding of simple record types.
  • Ascertain the selection of the correct principal and secondary diagnoses and procedure codes.
  • Determine proper sequencing of diagnosis and procedure codes.
  • Contact physicians or other health care professionals to obtain diagnostic information.
  • Code the medical record using an automated encoding system and coding manuals/references.
  • Assign correct APC or DRG assignments as appropriate with the assistance of the encoding system.
  • Abstract required data elements from the medical record to create a complete and comprehensive database.
  • Verify accuracy of patient demographic data fields to ensure data integrity and proper reimbursement.
  • Locate and utilize the necessary resources to resolve coding questions.
  • Refer all incomplete or ambiguous cases to the coding supervisor/department manager.
  • Work assigned claim edit and charge review work queues as needed.
  • Maintain current knowledge of ICD-10-CM and PCS and CPT/HCPCS coding systems.
  • Maintain proficiency in ICD-10-CM and ICD-10-PCS. Understand new coding guidelines.
  • Participates in ongoing education and training.
  • Other duties as assigned

Requirements:

Education and/or Experience
-
Currently enrolled in or successful completion of an accredited Health Information Technology.

  • Certified Coding program required. Current credential status preferred.
  • Coursework in Anatomy, Physiology, Medical Terminology, and Pathophysiology.
  • Coding experience in an acute care hospital or physician office setting preferred.
  • Computer proficiency required.
Experience with computerized encoder systems preferred.

  • CCS, CCA, RHIT, or RHIA credentialed status preferred.
