Lpn -clinical Documentation Specialist - Holyoke, United States - Holyoke Health Center
Description
This role focuses on enhancing the quality, accuracy, and completeness of clinical forms documentation:
- Obtain provider documentation from the Electronic Health Record for form completion
- Coordinate correspondence for provider review and educate clinical care team members on documentation guidelines
- Ensure forms are completed within 7-10 Business Days or communicate delays to patients
- Collaborate with HIM and clinical staff to improve form quality
- Interpret clinical information accurately for form completion, including CPT and Diagnosis codes
- Act as a liaison with patients for form completion, striving to meet timelines and assist providers
Requirements:
- Accredited Licensed Practical Nursing Program degree and current Massachusetts State Board of Registration license
- Minimum 1-3 years of LPN experience, preferably in an ambulatory setting
- Strong communication (verbal/written), organizational, problem-solving, and management skills
- Proficient in critical thinking, complex decision-making, and navigating electronic health records
- Knowledge of clinical documentation improvement, coding, and chart review is a plus
- Self-directed, independent, and flexible in work assignments

Pay: $30.00 per hour
Standard shift: Day shift
Weekly schedule: Monday to Friday
Ability to Relocate: Holyoke, MA 01040: Relocate before starting work (required)
