Ditmas Children's is hiring an ADMISSIONS COORDINATOR
JOB DESCRIPTION
- Review PRI's and all supporting clinical documentation for appropriateness to the facility.
- Serve as a liaison to hospitals throughout the metropolitan area, making field visits as needed and telephone consultations to confer with hospital staff and to seek out and evaluate potential applicants for admission to the facility.
- Complete all required clinical preadmission assessment forms on all potential evaluated, gathering additional medical and social information as needed for a thorough assessment.
- Document all applicant clinical and psychosocial assessments.
- Submit monthly statistics and reports as requested by the Director of Admissions.
- Contact Managed Care Companies to obtain preauthorization; review level requirements to obtain the appropriate level prior to admissions.
- Conduct tours of the facilities with potential patients, families, and/or representatives.
- Interact with HMO Case Managers prior to admissions, to review skilled needs and maximization of level.
- Provide weekend and holiday admission coverage as needed.
- Perform other duties as required.
Job Qualifications:
- RN, LPN, or Certified Social Worker registered in the State of New York.
- MDS experience and PRI certification would be a plus.
- Previous experience in postacute admissions processes.
- Acute Care experience preferred.
- Bilingual is a plus.
Specialized Skills and Competencies:
- Respond politely and helpfully to telephone and inperson requests for service consultations.
- Excellent writing and clinical assessment skills.
- Good working relationships with staff and referral services.
- Ability to multitask and work accurately in a fastpaced environment.
- Knowledge of Electronic Medical Record, Microsoft Outlook, Word, and Excel.
- Required to speak and write in an understandable manners.
Other Skills and Competencies:
- Ability to relate to children and their families in a manner that respects their needs and capabilities.
- Thorough knowledge and understanding of medical terminology, conditions, and treatments relevant to children and adolescents.
- Ability to make thorough and accurate biopsychosocial assessments of children and adolescents about the continuum of services provided by the facility

Compensation:
Competitive compensation ranging from $70,000- $101,000 per year.
