Monterey Park Hospital located in the San Gabriel Valley of Los Angeles, is a 101 bed acute care community hospital.

Monterey Park Hospital has provided our community with advanced medical and culturally sensitive healthcare services for over 50 years. Monterey Park Hospital is currently recruiting for a healthcare
Discharge Planner to work in our Utilization Review Department. This position reports to the Director of Managed Care


Responsibilities:


Qualifications:

Education:
High School Education/GED or equivalent preferred. Associate's/Technical Degree or

equivalent combination of education/related experience preferred.


Experience: 2 years' experience working in an acute care facility or similar healthcare position

preferred.


License/Certifications:
Basic Life Support (BLS) required.


Other:
Demonstrated knowledge of Medicare/Medi-Cal and HMO Health Plans required. Demonstrated

good communication skills, both verbal and written English, required. Bilingual English/Spanish to

effectively communicate with patients and families preferred. Ability to facilitate and lead

interdisciplinary rounds.
