Summary:
The Clinical Nutritionist counsels, educates and develops therapeutic nutritional plans and accompanying goals for patients. The Nutritionist is part of a team of providers within the Patient Centered Medical Home (PCMH).
This person will support CHP's mission, vision, and values and will adhere to compliance protocols as well as CHP's policies and procedures.
