As the Care Coordination Operations Support Specialist, you will have the opportunity to support the Care Coordination and system teams in Tucson, AZ.

This is an exciting role to supporting system initiatives, creating and developing new workflows and education to our frontline team members.

This is a full time opportunity. You will provide onsite support at one of the two Tucson, AZ facilities. There may be some onsite meetings in Phoenix, AZ at the corporate location. Training days may be in Phoenix, AZ as well. Expected hours are Monday through Friday, 8AM to 5:00PM.

This position is responsible for supporting Care Coordination leadership in the planning, development and supporting the on-boarding and orientation of newly hired Care Coordination employees, auditing and analysis for defined KPIs, gathering and analyzing data metrics as requested, development and/or execution of effective internal communications.

This role performs under the guidance and direction of the Executive Director or facility Director and serves as a liaison between all disciplines by effectively communicating, educating and problem solving.

This position proactively supports the quality and continuity of services throughout the healthcare systems and assists in resolution of systems issues and enforces facility/department policy/procedures and standards.

This position coordinates oversight of quality, risk management, safety and transition of care standards and is a role model and resource person.

Collaborates with Clinical Education to coordinate and support the on-boarding and precepting of newly hired employees.

MINIMUM QUALIFICATIONS


Requires a level of business and clinical knowledge as normally obtained through the completion of a Bachelor's Degree in business administration, health services administration or related field.

Requires two years of experience in an acute or post-acute care setting.

Knowledge of healthcare management as typically obtained through two years of progressive experience in healthcare operations.

Knowledge of the requirements of the various regulatory and accrediting agencies as applicable.


Requires critical thinking, communication, influence, decision-making, analytical, and flexibility skills to make optimal decisions based on multiple variables and desired outcomes.

Must have the ability to practice skills according to department and professional standards and quality requirements.

BLS required. BLS not required in post-acute setting.
