Duties and Responsibilities:

Performs diagnostic initial assessments, comprehensive assessments, SRA's, Initial ISP's, implementing treatment plans, CEPP, care coordination, crisis response, discharge planning, and continued stay for clients.

Provide outpatient therapy services to adults and children and prepares required session notes within 24 hours of providing services in the office and community.

Participate in the development and implementation of groups in the community

Must be available and participate in a once per month weekend rotation providing crisis services to clients. Will provide coverage in absence of availability.

Provides case management functions of linking, arranging, and advocating for services and referrals for clients.

Maintain up-to-date client records to include reports and required session notes in the EMR in a timely and accurate manner.

Reviews progress notes, quarterly reports, and ISP's to ensure clients are working towards goals and objectives, and that documentation is clinically sound and aligned with the agency, DBHDS, and Medicaid guidelines.

Works in various departments, as needed, including Crisis Stabilization, Community-Based Services, and Substance Use services, as one's skill sets allow.

Perform other job related duties, as assigned.
