Description

This role is crucial in providing essential information about our hospice services to potential patients and their families, managing patient admissions, and ensuring thorough care from the initial assessment to ongoing case management.

• Educate potential patients and families about hospice services.
• Efficiently handle patient admissions and complete all necessary documentation.
• Perform comprehensive nursing assessments and create initial care plans.
• Implement nursing interventions and provide ongoing case management.
• Conduct routine visits, manage medications, and monitor patient status.
• Ensure documentation, including Plans of Care, HIS questionnaires, and medical orders, is completed accurately and submitted within 24 to 48 hours.
• Participate in IDG Meetings and communicate right away patient issues.

• Current Texas RN license with experience in hospice care preferred.
• Proficiency in Kinnser EMR software, with strong knowledge of completing Plans of Care, HIS questionnaires, and orders.
• Excellent organizational and communication skills.
• Flexible and able to work effectively in a dynamic team environment.
• Committed to providing compassionate and patientcentered care.
• Be available oncall when need.
