- Collaborates on the comprehensive care plan with other healthcare professionals involved in the patient's treatment and ensures seamless continuity of care by coordinating and sharing information with the physician, agency staff, patient, family, and community resources—all within a Point of Care environment.
- Performs the initial and/or thorough nursing assessment, identifies the primary care focus, devises the care plan based on state-specific regulations, and submits accurate, thorough, and timely documentation in line with physician directives.
- Conducts comprehensive patient assessments at specified intervals and formulates the appropriate care plan.
- Verifies that patients meet the eligibility criteria for home health services and the medical necessity standards set by the payer source.
- Active RN license in the state of practice
- Current CPR certification
- Valid Driver's License, vehicle insurance, and access to a reliable vehicle or public transportation
