Director, Health Services (RN Case Manager) - Home Health


Are you a Licensed Physician or an RN who wants to be part of building something from the ground up? Do you want to be part of the change that makes a difference in the lives of individuals who need our help and support? ULife Health is searching for someone who can fill the role of Director, Health Services.

As we are getting started (we are a startup) this role is best for someone looking for fractional/part-time/PRN work. As we grow, so will this role, and full-time will be available. This is a hands-on role that may require on-site home visits and skilled medical tasks to be conducted.


The Director, Health Services is critical to the organization and will be part of the advisory board which will provide you the opportunity to help develop, implement, and drive policies and procedures that improve patient care and patient outcomes and increase organizational efficiencies.

Be The Change Be The Difference

  • Work with team members to develop, implement, and maintain organizational policies and procedures
  • Supervise and train RNs, LPNs, CNAs and HHAs
  • Assign tasks as necessary to RNs, LPNs, CNAs and HHAs
  • Conduct patient assessments and reassessments (including patient environments)
  • Perform Quality Management tasks
  • Communicate with medical patients and nonmedical care clients during the Continous Care Cycle as part of the ULife Health Compassionate Care System
  • Collaborate to develop and implement nonmedical care plans
  • Collaborate with Primary Physicians to develop and implement medical care plans
  • Review patient health care directives
  • Coordinate care when multiple services are needed
  • Perform basic home health procedures including blood pressure measurements, administering injections, administering medication, EHR patient data and visit data entries, etc
  • Provision and coordinate therapy (i.e. PT, OT) with existing home health services when needed

Your Education & Experience Are Needed

  • Licensed Physician with 2+ years of Home Health experience OR
  • RN with 3+ Years experience, 2+ Years Home Health experience, and 1+ Year Supervisory experience
  • Knowledge of applicable local, state, and federal laws.

Pay:
$45, $85,000.00 per year
