Responsibilities:


  • Report patient care/condition/progress to patient's physician and Clinical Manager on a continuous basis.
  • Follows infection control standard precautions and uses personal protective equipment as required.
  • Implement patient care plan in conjunction with patient and family to assist them in achieving optimal resolution of needs/problems.
  • Discharge patients after consultation with the physician and Clinical Manager, preparing and completing needed clinical documentation.
  • Prepare appropriate medical documentation on all patients, including any case conferences, patient contacts, medication order changes, recertifications, progress updates, and care plan changes. Prepare visit/shift reports, updates/summarizes patient records, and confers with other health care disciplines in providing optimum patient care.
  • Confer with physician in developing the initial plan of treatment based on physician's orders and initial patient assessment. Provide handson care, management and evaluation of the care plan and teaching of the patient in accordance with physician orders, under Clinical Manager's supervision. Revise plan in consultation with physician based on ongoing assessments and as required by policy/regulation.
  • Administer ongoing care and case management for each patient, provide necessary followup as directed by the Clinical Manager.
  • Provide admission, case management, and followup skilled nursing visits for home Health patients.
  • Exhibits an attitude which promotes harmony and goodwill in the workplace.
  • Promotes quality, comprehensive services through a team approach.
  • Performs other duties as assigned.

Education and Experience:


  • Must have current or compact licensure in the state of Texas.
  • Valid Driver's License and Automobile Insurance. Pass the Background check.
  • At least
    two years of experience, preferred, as a Home health Nurse in a hospital, nursing home, home health agency, hospice, or long term care facility.

Skills and Abilities:


  • Shall have a genuine interest in geriatrics, preferably, shall have experience in geriatric care.
  • Must be able to assess, evaluate and communicate effectively.
  • Must demonstrate sound knowledge and actions in patient care and decision making.
  • CPR certification required, other medical certification highly encouraged (i


e:

PICC line, IV therapy, chemotherapy, etc.)

  • Participates in continuing education to broaden knowledge and acquire skills necessary to be effective in all aspects of job performance.
  • Must have reliable transportation and a valid driver's license.
  • Able to fulfill shared oncall requirements with immediate 24/7 availability when assigned.
