    OVERVIEW

    The position provides case management services to children who have been identified as seriously emotionally disturbed who are at risk for out-of-home placement and/or have multi-agency involvement. Services are provided in the home, office, community, schools, juvenile detention facility and residential placements. The primary goal is maintaining youth from ages 4-21 years of age in the least restrictive environment that the youth can be maintained. At times, the least restrictive environment may be a residential setting. in that case, the case manager monitors progress by doing on-site visits with the individual regardless of where they are located in Virginia, participation in treatment team meetings and gathering updates from on-sight staff The case manager is responsible for discharge planning to transition the individual back into the home community and establish wrap around services to support the child and family upon the child's return. This position also acts as the liaison to Commonwealth Center for Children and Adolescents (CCCA). Case Manger provides discharge planning for all children admitted from Caroline County, Fredericksburg City, King George County, Spotsylvania County and Stafford County. Monitoring requires participation in treatment team meetings with CCCA, referrals to services in the community, meetings with parents/guardians in the community, face to face meeting with the child upon discharge, securing funding for services and some on-site visits to CCCA.

    DUTIES AND RESPONSIBILITIES

    1. Conducts intake interviews, obtains data and develops diagnoses and treatment plans.
    2. Maintain required agency paperwork and filing.
    3. Monitor individual progress through family and collateral contacts.
    4. Prepare for Family Assessment Planning Team (FAPT) meetings. Assessments of needs, identify needed services, identify proper vendor, complete CANS, complete FAPT paperwork and attend meeting.
    5. Attend meetings with Court Services Unit or the schools for IEP's, Behavioral Support Plans, Discipline meetings, Treatment team meetings.
    6. Identify residential placements, complete the application packets, secure funding, gain required signatures for paperwork, coordinate admission, and attend admission.
    7. Monitor individuals in residential placements ~ treatment team meetings, follow up with staff, follow up with family, residential visits, complete discharge planning.
    8. Participate with Commonwealth Center for Children and Adolescents in discharge planning for all individuals from Planning District 16.
    9. Helps identify and link individual to resources and services within the community.
    10. Meets face to face with individual upon discharge.
    11. Complete required yearly trainings.
    12. Substitute as Family Assessment Planning Team member.
    13. Perform other duties as assigned.

    KNOWLEDGE, SKILLS, AND ABILITIES

    Thorough knowledge of the diagnosis and treatment of the seriously emotionally disturbed child/ adolescent; related family and individual treatment; crisis intervention techniques; intervention techniques, such as behavior management, independent living skills training, supportive counseling, family education, crisis intervention, discharge planning, and service coordination. Thorough knowledge of principles of child and adolescent psychology; physical and intellectual development and; principles of family and group dynamics; principles of individual oriented services, empowerment of individuals and community based treatment for individuals from birth to age eighteen and families. Thorough knowledge of individual management guidelines for Virginia, good knowledge of office management practices, ability to maintain individual records and ability to protect individuals' rights. Knowledge of local community resources and service delivery systems, including support services, eligibility criteria and intake processes, termination criteria and procedures, and generic community resources. Skills of effective oral, written and interpersonal communication, interviewing, observing, recording and reporting on individual's functioning. Skill in identifying and documenting a individual's needs for resources, services and other supports and in using information from assessments, evaluations, observation and interviews to develop service plans. Skill in identifying services within the community and established service system to meet the individual's needs. Skill in negotiating with individuals and service providers and coordinating the provision of services by diverse public and private providers. Skill in identifying community resources and organizations and coordinating resources and activities. Ability to establish and maintain rapport with the individual population and function calmly in stressful situations with individuals and families. Ability to demonstrate a positive regard for individuals and their families; empowering families to meet the needs of their children; being persistent and remain objective in providing assistance to individuals. Ability to work as a team member, develop and run treatment teams, maintaining effective inter and intra agency working relationships, communicate effectively with supervisors and administrative staff and communicate effectively with Family Assessment & Planning teams and Community Policy and Management teams. Ability to work independently, performing position duties under general supervision and communicate effectively, verbally and in writing. When the training is available, complete training in the High Fidelity Wraparound model. Provide Intensive Care Coordination, when requested by CPMT, with one hour a week of clinical supervision from a licensed therapist. The licensed therapist or administrative supervisor shall be trained in the High Fidelity Wraparound Model.

    TYPICAL EDUCATION, TRAINING AND WORK EXPERIENCE

    Bachelor's Degree in the Behavioral Sciences/Human Services and equivalent education/ experience that provide the required knowledge, skills, and abilities. Master's Degree in the Behavioral Sciences/Human Services preferred.

    LICENSURE AND CERTIFICATIONS

    Valid VA driver's license. State professional licensure or eligibility for same preferred.
