Job Overview:
We are seeking a compassionate and dedicated Medical Social Worker to join our team. As a Medical Social Worker, you will provide support and assistance to individuals and families facing medical challenges. Your role will involve assessing patients' psychosocial needs, coordinating care plans, and connecting patients with appropriate community resources. Responsibilities:

Conduct comprehensive assessments of patients' psychosocial needs
Collaborate with healthcare professionals to develop individualized care plans
Provide counseling and psychotherapy services to individuals and families
Assist patients in accessing community resources and support services
Advocate for patients' rights and ensure their needs are met
Maintain accurate and uptodate medical documentation
Collaborate with interdisciplinary teams to coordinate patient care
Provide education and support to patients and their families regarding medical conditions and treatment options
Conduct individual and group counseling sessions as needed

Qualifications:

Master's degree in Social Work or related field
Valid state licensure as a Social Worker
Experience working with senior care
Knowledge of medical documentation standards and ICD10 coding
Proficiency in providing psychotherapy and counseling services
Strong communication and interpersonal skills
Ability to work effectively in a multidisciplinary team setting
Knowledge of child and family counseling techniques is desirable
Reliable Transportation. Valid Auto Liability Insurance

This is a PRN contract position. Rate of pay and mileage reimbursement is negotiable upon discussion with the Administrator.
