    Position Summary:

    This position will work with our clients to locate affordable housing units through the use of community

    resources and agency network. Depending upon assignment, the majority of duties may include, but are not limited to:

    Essential Functions:

    • Assess housing barriers of individuals and families to determine housing and service needs.
    • Assist chronically San Leandro and Alameda based homeless clients and assess housing barriers of individuals to determine housing and service needs.
    • Utilize a housing stability plan to assist with establishing and meeting housing goals and identifying appropriate steps towards achieving goals, including modifying goals as needed, and evaluation of progress.
    • Assess and screen household for program(s) eligibility.
    • Work in partnership with the assigned case manager and client on housing search and maintaining secure housing.
    • Meet with families and individuals to evaluate income and expenses and determine their ability to pay rent in the future.
    • Promote values of self-sufficiency and empowerment throughout work with clients.
    • Promote and facilitate active participation by clients in program services.
    • Provide housing/tenant counseling, housing workshops and community resource referrals.
    • Serve as an ongoing liaison between landlords and participants.
    • Educate/advocate around budgeting and financial management practices, including, banking and credit repair.
    • Support a caseload of individuals, which will include intensive home-based case management collaboration with the referrals case manager.
    • Serve as an ongoing liaison between landlords and participants.
    • Maintain accurate daily logs, case notes and files for all clients.
    • Collect and enter program data as needed including but not limited to HMIS.
    • Attend and participate in scheduled case review meetings, staff meetings and training.
    • Must be available to work Monday-Friday and some occasional evening hours.
    • Other duties as assigned.

    Skills & Abilities:

    • Ability to build supportive and respectful working relationships with individuals diagnosed with a mental illness that instills hope and promotes self-determination.
    • Sensitivity to and understanding of the special needs of the homeless.
    • Proven ability to work independently and as an effective and collaborative member of a team.
    • Possess an understanding of and practice cultural sensitivity through open dialogue and self exploration with diverse groups, while providing direct services.
    • Ability to effectively intervene in crisis situations, with de-escalation techniques.
    • Bachelor's Degree or Equivalent Professional experience in the human services or related field and demonstrated experience in rental housing programs for low income, special needs individual and families.
    • Minimum of three years of experience working in housing field with strong knowledge of the
    • Alameda County housing market and geography.
    • Must possess 1 year of field experience working with people with serious mental illness, individuals with a dual diagnosis and/or the homeless population.
    • Knowledge of federal, state and local fair housing laws.

    PHYSICAL REQUIREMENTS & WORKING:

    The work environment and physical demands described are representative of those required by employees to perform to the essential functions of this job with or without reasonable accommodation.

    • Ability to work at a desk or computer workstation.
    • Ability to perform repetitive office tasks.
    • Ability to review a wide variety of materials in electronic or hard copy form.
    • Sufficient manual dexterity to enable the employee to operate a personal computer, phone, and related equipment.
    • Must be able to lift 25 pounds.
