Clinical Department Assistant - Spring Valley, United States - TRI-COUNTY CARE LLC
Description
Essential Responsibilities:
- Serve as part of the Clinical Team for Person Centered
- Assist the clinical team with coordinating and scheduling triage
- Responsible for weekly reconciliation of cases referred to the clinical team by incident
- Conduct weekly reconciliation of SQL reports of clinical team referrals made in
- Update CM team with any followups regarding the
- Proactively monitor files to ensure that weekly updates are coming
- Assist with some nursing tasks that require followup, when needed (i.e. hospitalization )
- Participate in mandatory staff meetings and take meeting minutes, as
- Distribute the meeting minutes once the recommendations are sent out by VP of Clinical
- Other duties as assigned/requested.
Specific Knowledge, Skills, and Abilities:
- Knowledge of developmental disabilities, chronic disease and social determinants of
- Ability to engage people with behavioral health
- Knowledge of personcentered planning
- Demonstrates effective leadership
- Demonstrates cultural
- Demonstrates ethical and professional responsibilities and
- Demonstrates capacity to use Health Information Technology to facilitate
- Knowledge of confidentiality regulations/HIPPA.
- Ability to prioritize and complete work in a timely
- Proactively approaches professional
- Demonstrate a comfort level of engaging with people with behavioral health
Experience:
- High School Diploma
- General knowledge about mental health field,
- Knowledge about OPWDD services and regulations
- Strong computer
- Strong Organizational
- Familiarity with personfirst, personcentered planning
