    This position operates within the Healthcare Homes program, managing hospitalization processes and addressing gaps in care for our highest-need population.

    Additionally, you will serve as a valuable resource for clients seeking support with smoking cessation services.

    Key Responsibilities:

    • Assist in developing wellness and prevention initiatives Collaborate with the PCP Consultant to identify gaps in treatment for clients on the high utilizer report Outreach clients on the high utilizer report to develop plans to address frequent hospitalizations Remain a resource for clients desiring smoking cessation support Assist in outreach and enrollment for the HCH team Provide clerical support to the HCH team as needed Implement required DMH Net Health technology programs such as Cyberaccess and CareManager Assist with tracking hospitalizations and communication with the treatment teamEducation and/or

    Experience Qualifications:

    • Degree in Nursing (LPN or RN), Counseling, Social Work or other social services field with at least 2 years of experience Preferred knowledge of community resources

    Additional Qualifications:
